How do I obtain OPTION-v™ valved urinary catheters?
Take this information to your physician to determine if OPTION-v catheters should be part of your care. Your doctor’s office can order directly from Opticon by calling 614-336-2000, or the doctor may provide you with a prescription. (See attached order/prescription form.)
Are these products covered by my health insurance?
Insured patients may consider purchasing OPTION-v catheters from CCS Medical, our “direct-to-home” pharmacy partner. CCS Medical will ask for your insurance information and will help determine your coverage. CCS Medical can be reached at 888-724-4357.

Have your doctor’s office fax the completed order/prescription form to us at 888-336-9258, or you can mail the completed original order/prescription form directly to us.
